To Schedule an Appointment
Please call 715.842.4288 or toll free 800-210-6223.
Weekdays between 8:00 am and 5:00 pm. Please let the Medical Assistant know what
concerns you wish to address so we can schedule the appropriate amount of time to meet
your needs.
Office Visits
Patients are asked to arrive 15 minutes before the scheduled appointment time so we can
obtain medical history and insurance coverage information. Please bring prescription and
over-the-counter medications and any supplements you are taking to your appointment. A
parent or guardian should accompany patients under the age of 18. Allow a minimum of
one hour for regular office visits. Out of courtesy for others, patients arriving late may be
asked to reschedule.
Appointment Cancellations
If you are unable to keep a scheduled appointment, please let us know at least 24 hours in
advance so we can offer that appointment time to other patients. We regret that failure to
present for a scheduled appointment repeatedly may result in discontinuation of our
services.

input from the people we value most, our patients.
